What you will do:
* Facilitate safe and timely hospital discharges by assessing patients' social care needs and devising comprehensive discharge plans.
* Collaborate with multidisciplinary teams, including medical staff, families, and external agencies, to ensure cohesive care planning.
* Conduct assessments and reviews, ensuring compliance with relevant guidelines and frameworks.
* Provide emotional and practical support to patients and their families, navigating through challenging transitions.

This role must align with MOC guidelines including:
* Candidates must have two years of post-qualified experience.
* Applicants should have refrained from permanent social worker roles for the past 12 months.
