Job Summary
We have a full‑time vacancy for a Social Worker within the Eastbourne & Conquest Hospital Community Team, based at St Mary’s House in Eastbourne. In this role you will assess adults who have been discharged from hospital and support them with ongoing care planning to maximise their independence.
Key Responsibilities
You will hold detailed knowledge of the Care Act (2014), Mental Capacity Act (2005), Equality legislation and the Deprivation of Liberty safeguards, and will implement this in your daily practice. You will also carry out Local Authority safeguarding duties to protect and mitigate risks to the individuals we serve. You will collaborate with local partners, hospital trusts and Integrated Commissioning Boards (ICB) to secure continuing care funding and ensure good outcomes for the individuals.
