Job summary
The postholder would mainly work with the South of Tees Urgent Care service based at Roseberry Park Hospital. They will also work closely with their counterpart for the North of Tees Urgent Care Service. The postholder will demonstrate the flexibility of working with the North of Tees Urgent Care Service as and when a need arises, on a reciprocal basis. The post also covers the Crisis Assessment Suite based at Roseberry Park Hospital; this work is shared with the Consultant for North of Tees Urgent Care Service
The Urgent Care Service aims to ensure that a high quality alternative to acute psychiatric admission exists for those individuals in the locality who are experiencing a crisis related to severe mental health problems. The service is gatekeeper to all Inpatient beds to allow consideration of alternatives to Inpatient care. The service also aims to provide an effective In Reach Service to facilitate early discharge from hospital. A member of the Crisis/IHT Team attends the Inpatient 24 Hour Report out providing clear communication links between the two services. A member of the Crisis/IHT Team attends Discharge Planning Meetings and works with the Care Coordinator to facilitate discharge. It is a 24 hour, 365 day per year service and is capable of a rapid response seeing all referrals within 4 hours. It is a multidisciplinary team comprising staff from a variety of disciplines.
Main duties of the job
It is anticipated that the average number of inpatients that the Consultant will be involved at any point in time is 10-14 with RC responsibilities but with Speciality Doctor support. There is enough support and supervision available and good level of MDT working. Average length of stay for our inpatients is 35 days but there is a clear aim to reduce this to match national recommendations of bed occupancy.
There is an expectation that the Consultant will attend team daily huddles which is where patient are discussed, Consultants will be required to support the team on a day to day basis and review patients as and when necessary. The Consultant will also attend weekly Supercell meeting (attended by Team manager, Consultant, Advanced practitioner, Psychologist to discuss team and patient related issues.
