Care Coordinator – Royal Primary Care Chesterfield, part of Chesterfield Royal Hospital NHS Foundation Trust
We have an opportunity for 1 x part-time (22.5 hours per week), Care Coordinator based at Royal Primary Care East sites, based in Chesterfield.

Job responsibilities
* To work within the Administration team with a focus on achieving excellent patient care via the telephone and face to face.
* The successful candidate's main responsibilities will be to manage the registrations and prescription management of a busy respite care home.
* Provide professional and holistic administration support for all patients who are discharged to the home, ensuring a smooth and safe transition from hospital to respite to home.

Person Specification
* Experience – Essential: Experience working in a pharmacy, healthcare or general practice setting; experience of managing clinical administrative tasks/workload; experience of working within a multi-professional team. Desirable: Experience of registering new patients; experience of managing patient prescriptions.
* Skills and Knowledge – Essential: Ability to work under pressure and prioritise workload; ability to manage multiple tasks in a fast-paced environment; able to work independently or as part of a team; good interpersonal and customer service skills; good written presentation skills; able to communicate effectively with a wide range of stakeholders. Desirable: Knowledge of clinical systems (e.g., SystmOne); knowledge of NHS prescription and medication guidelines; understanding of Electronic Prescribing Service (EPS).
* Qualifications and Training – Essential: GCSEs in Maths and English or equivalent qualification. Desirable: GP Assistant Certificate qualification; NVQ Level 2 (or equivalent) in Pharmacy Services or related healthcare qualification or willing to work towards.
