Key Details:
* Role: Lead Complex Case Manager (Band 8a)
* Location: Hybrid Working (Home/Patient Facing)
* Hours: Monday to Friday, 9am – 5pm (37.5 hours per week)
* Contract: Initial 6-month term
* Patient Interaction: Yes, occasional visits across Birmingham and Solihull
Responsibilities:
* Manage complex patient cases, ensuring high-quality care and support.
* Coordinate with healthcare professionals and service providers.
* Conduct patient visits as necessary.
* Develop and implement care plans tailored to individual patient needs.
* Provide leadership and guidance to case management teams.
Requirements:
* Proven experience in complex case management within the NHS or similar healthcare settings.
* Strong leadership skills and the ability to manage and support teams.
* Excellent communication and interpersonal skills.
* Ability to work independently and manage time effectively.
* Registered Nurse or relevant healthcare qualification.

* Professional Qualification and current registration with relevant professional body i.e. NMC/HCPC (Nurse/SW/OT/AMHP)
* Educated to degree level or equivalent level of experience working in a specialist area
