Job Overview
We are looking for a Liaison & Diversion Support, Time, Recovery Worker based in Hereford. This role is not eligible for visa sponsorship, and may be withdrawn if sponsorship is required.
Main Duties of the Job
*THIS ROLE IS SUBJECT TO POLICE VETTING*
The post holder will be expected to travel across the MPFT footprint if required.
 * Work with service users referred to Health & Justice and their carers to promote self‑determination, choice and autonomy.
 * Establish therapeutic relationships with service users and their relatives/carers.
 * Participate in setting quality standards, including auditing, monitoring and reviewing practice in line with current clinical guidance and policy.
 * Assess a person’s individual care needs, agree and refer to appropriate agencies, and involve them in planning and prioritisation of their activity where possible.
 * Provide short‑term interventions to encourage engagement with services and attendance at initial appointments.
 * Complete robust risk assessments and risk management plans, and promote positive risk taking to improve quality of life and maximise occupational performance/independence.
 * Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
 * Utilise agreed outcome measures to review the effectiveness of the process, as part of the Health & Justice Service.
Person Specification
Qualifications & Training
Essential criteria
 * Competent in literacy and numeracy.
 * Computer literacy.
Desirable criteria
 * NVQ Level 2 Health & Social Care or equivalent.
 * Training in Supporting / Mentoring Adult / Child Safeguarding.
Experience
Essential criteria
 * Experience working with individuals with a complex range of health and social care needs.
Desirable criteria
 * Experience of the Immigration system.
Skills, Knowledge & Abilities
Essential criteria
 * Basic knowledge of physical/mental condition and rehabilitation.
