An exciting opportunity has arisen for a Social Worker to join our enthusiastic Home Safe Hospital Discharge Service at Wansbeck General Hospital.
While the position is primarily based at Wansbeck General Hospital, the successful candidate must be flexible to provide cover at other hospitals across the County, including Blyth Community Hospital, Northumbria Specialist Emergency Care Hospital, and Newcastle Hospitals.
The role is for 37 hours per week on a permanent contract.
The Role:
You will collaborate with individuals, families, and carers, utilizing interpersonal and communication skills to assess complex psychosocial needs, set objectives, develop care plans, and deliver interventions to ensure safe hospital discharges. Responsibilities include completing Urgent Needs Assessments, Mental Capacity Assessments, and working with external services to access necessary resources.
You will manage your caseload with a good balance of autonomy and support, with regular supervision and full support from your Team Manager.
It is essential to have a clear understanding of the Care Act and the statutory duties of Local Authorities. We are committed to safeguarding vulnerable adults, and all staff are expected to share this commitment.
The salary for this part-time position ranges from £38,626 to £42,708 pro-rata.
