Exciting new opportuinity tojoin theMulti-Disciplinary Team as adedicated and experienced Social Worker in Midhurst. You will be based at a newly established neurological centre, providing care for men and women over the age of 18 with acquired brain injuries. Teamwork is at the heart of this role, ensuring a collaborative approach to care.
Working 37.5 hours per week, you will be part of a multidisciplinary team, working closely with the Hospital Director and other professionals. You will build strong relationships with service users, supporting them in their treatment plans and helping them work towards a brighter future. Additionally, you will gather and manage information related to service users and their care plans.
With prior experience in either the NHS or private care, you will oversee a complex caseload, conducting social care assessments, risk assessments, and facilitating group activities. You will also contribute to ICR meetings, helping service users create realistic and meaningful plans for their future.

Key Responsibilities:
* Conduct thorough social care assessments and prepare reports from pre-admission through to discharge.
* Act as the main point of contact for home authorities from the admission stage, maintaining this connection throughout the service user’s stay.
* Work closely with home authorities and service users to support families in the decision-making and care planning process.
* Assist in preparing reports for Mental Health Review Tribunals and appeals to MH Act Independent Hospital Managers, and attend hearings as required.

What We’re Looking For:
* Registered with Social Work England
* Completed ASYE (Assessed and Supported Year in Employment) programme
* At least 2 years post-qualification experience, preferably within a similar mental health or care environment.
* Experience working with young adults, particularly in relation to eating disorders or complex mental health needs, is highly desirable.
