About the Role
Livewell South West is looking for OOA Prevention Nursing Assistants to support patients who require 1:1 care. The role involves assisting trained nurses with physical, social and psychological care for patients/clients with mental health problems who are admitted to an LWSW mental health unit. The primary focus is to support patients during acute illness to prevent them being sent out of area and to enable care close to home.
Main Duties
* Work within all Mental Health wards of LWSW, including assignment across services on short notice.
* Adapt to flexible shift patterns, including nights, to provide 24/7 service (e.g., 7–3pm, 9–5, 12–8pm, 8–8pm, nights).
* Support patients on a 1:1 basis, assist with risk management and therapeutic activities as directed by the registered nurse.
* Inter-ward escort of patients to other wards, departments, or community locations as directed.
* Assist the registered nurse in carrying out tasks that meet the patient’s physical and psychological needs, as identified in the care plan or assessment.
* Contribute to assessment, planning, implementation and evaluation of patient care, including participation in case reviews and admission/discharge processes.
* Maintain accurate records in the multidisciplinary patient file / electronic records, with counter‑signing by a registered nurse.
* Record and manage patient observations and personal care as outlined in the care plan.
* Handle patient property responsibly.
* Carry out responsibilities designated by the Ward Manager and Matron.
* undertake mandatory training as directed by the Trust and participate in continuing role/personal development using the appraisal system.
* Provide line management support and supervision, and contribute to any audit processes.
* Work in accordance with Livewell South West policies on health, safety, fire procedures, record keeping and confidentiality.
* Participate in the annual competency assessment and appraisal reviews.
* Ensure confidentiality and be aware of health and safety, mental health act, and safeguarding principles.
Experience
* Working within a Mental Health or NHS setting or equivalent.
* Experience of OPMH or multidisciplinary team working.
* Supporting individuals with eating disorders and complex needs.
* Awareness of vulnerable adult / protection issues and guidance.
* Recent experience of working in a care setting.
* Knowledge of clinical governance.
* Ability to work with minimum supervision.
* Ability to undertake baseline physical observations.
* Evidence of continuing professional development.
* Experience in a variety of Mental Health settings and multidisciplinary teams.
* Experience of supporting individuals to identify and work toward personal goals in a related social care or health setting.
Qualifications
* NVQ level 3, QCF level 3 in health and social care or equivalent qualification.
* Care certificate.
* Other evidence of training or development relevant to mental health settings.
* Ability to participate in activities with patients and contribute to high‑standard care.
* Ability to travel independently and timely between Livewell sites and community locations.
* Vocational Qualification Level II or evidence to that level.
* ECG and venepuncture competence.

Knowledge
* Knowledge of mental health and a range of therapeutic approaches, including working with learning disabilities.

* Resilience, risk management, and the ability to share personal experiences appropriately.
* Knowledge of government initiatives, the triangle of care, CPA, and solution‑focused approaches.
