What you will do
* Conduct comprehensive assessments and manage an allocated caseload, escalating complex cases as necessary.
* Collaborate with experienced practitioners to consolidate skills and knowledge while providing crucial support to those at risk, including adults and children.
* Work closely with individuals needing care and support, their families, and carers, to identify risks and develop strategies to minimize them.
* Design and implement care and support plans, education plans, and other necessary plans in partnership with relevant services.
* Apply social work techniques to meet the needs of clients and promote positive outcomes through professional support, advice, and effective decision-making.
* Participate in a duty rota system, responding immediately to urgent cases, conducting assessments, and planning crisis response actions.
