Main duties of the job
The post holder will manage chronic disease areas within the practice, conducting clinical medication reviews, particularly for patients with complex polypharmacy. They will support general practice staff with prescription and medication queries, assist with repeat prescribing, handle acute prescription requests, and perform medicines reconciliation for patients post-hospital discharge.
They will provide expert advice on medicines to patients and the multidisciplinary teams. Responsibilities include leading medicines optimisation, quality improvement, and overseeing aspects of the quality and outcomes framework and enhanced services across all sites. The post holder will also facilitate integration with community and hospital pharmacies to enhance patient care.
This role is key in improving care quality and operational efficiency, requiring motivation and passion to deliver excellent services. The post holder will be supported to develop into a non-medical prescriber as needed. They will work independently and as part of the Pharmacy Team, recognising the roles of colleagues and other stakeholders, including patients, senior and peer colleagues, and professionals from other organisations (e.g., CCGs). The post holder must acknowledge their limitations and refer to others when necessary.

Knowledge / Skills
* Excellent written and verbal communication skills
* Good written and verbal communication as well as adequate presentation skills
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.

* Demonstrates accountability for delivering professional expertise and direct service provision.

* An appreciation of the nature of GPs and general practices. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
Qualifications
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
* Independent prescriber or working towards /intent of gaining independent prescribing qualification
* Completed or undertaking the CPPE general practice clinical pharmacist training pathway
Experience
* Minimum of 2 years post qualification experience.
