As a key part of our community service, you will: Provide routine and urgent consultations for care home residents, housebound patients and frail or elderly individuals Complete comprehensive care planning Manage long-term conditions within community patients Work closely with our multidisciplinary team including, GP's, nursing, therapy, pharmacy and social care professionals Support palliative and end-of-life care planning Collaborate with care homes and families to provide coordinated care.
