About the Role
Greenwich ALD - The post holder will be part of the Mental Health and Challenging Behaviour Team and will undertake clinical assessments of patients with mental health and contribute to care plans accordingly.
They will conduct a minimum of 4 Mental Health out‑patient clinics per week. The caseload will be between 80 and 100 Mental Health (including Challenging Behaviour and Dementia) patients. Diagnoses will include psychosis, anxiety, OCD, affective disorders, dementia, autism, ADHD, etc.
Service Overview
The ALD service provides specialist health care and services for people over the age of 18 with learning disabilities living in Greenwich, Bromley, and Bexley boroughs. The teams aim to provide high quality, evidence‑based assessment, monitoring and intervention to support people with learning disabilities to become and stay well, independent and to live a meaningful life. Service users are empowered to be active participants in their own care and in service development.
Key Responsibilities
* Conduct clinical assessments and contribute to care plans for patients with mental health and learning disability.
* Deliver a minimum of 4 Mental Health out‑patient clinics per week.
* Manage a caseload of 80–100 patients, including challenging behaviour and dementia.
* Diagnose conditions such as psychosis, anxiety, OCD, affective disorders, dementia, autism, ADHD, and others.
* Collaborate with the pathway management team to support design and development of the pathway’s future.
