Adults Community Resource Team Social Worker

Caerphilly Social Services are looking to appoint an experienced social worker to join their CRT service. The contract is offering an immediate start with Monday–Friday hours and in the main you will be peripatetic across the various hospital sites with paperwork being written up at home. Pay rate for this role is set by the LA at £37.09 per hour. When working in the office, this is based at Ystrad Mynach. Part time candidates can be considered.
Responsibilities
The team covers a wide range of hospitals but mostly the Aneurin Bevan hospitals including YYF (Ystrad Mynach), RGH, The Grange, St Woolos, Neville Hall, YAB (Ebbw Vale) and other ABUHB hospitals. Potentially the team could cover Prince Charles Hospital, Merthyr – the RCT hospitals and Cardiff hospitals including UHW and Velindre.
The main focus is on hospital discharges and support post-discharge as the team follow their patients home for a short term period. You may also be required to support other team members within the CRT / Frailty and will visit with OTs / Physios to provide support around mental capacity and more complex social situations. The main remit includes mental capacity assessments, long-term care placements, CHC, etc.
Requirements
* A minimum of 6 months experience of working in the UK as a registered social worker
* A relevant Social Work Qualification
* Registration with Social Care Wales / Social Work England
* Enhanced DBS, registered on the update service or be happy to apply for a DBS
* 3 years of referencing
* Driving license with access to a car
