We are seeking an experienced and highly motivated Children & Young Peoples Continuing Care Nurse to join our Wolverhampton team. You will play a pivotal role in assessing, planning, and co-ordinating care for children and young people with complex health needs, ensuring delivery of high-quality, personalised, and family-centred services.
You will work within the NHS Continuing Care framework to:
1. Conduct holistic assessments with highly complex health needs to make evidence-based eligibility recommendations.
2. Develop and review care packages in partnership with families, education providers, social care, and other health professionals.
3. Source and commission tailored support packages.
4. Provide compassionate, sensitive communication to children, young people, and their families, including delivering complex information and managing expectations regarding care provision.
5. Offer expert advice and guidance to the All-Age Continuing Care team, ICB, and wider health and social care community.
6. Ensure that care meets the highest standards of safety, quality, and compassion.
You will be a Registered Children’s Nurse (RSCN/RN Child) with substantial post-registration experience, including working with children & young people with complex and long-term health needs. You will have a thorough understanding of the National Framework for Children and Young People’s Continuing Care and be confident working across organisational boundaries to achieve the best outcomes.
