We are looking for a dedicated and motivated Social Worker to join our dynamic Adult Social Care Hub, the front door to vital care and support services in our community.

Our Hub is where meaningful change begins. You will be at the heart of safeguarding, triage, assessment and early intervention, making timely, strengths-based decisions that transform lives.

Our Adult Social Care service is built on highly skilled, supportive multidisciplinary teams who share a strong “can do” and “no wrong door” ethos. Working as one integrated service, we consistently go the extra mile to ensure residents receive the right support at the right time.

We work in close partnership with health services and wider agencies to keep people safe and well within their communities, helping to prevent unnecessary hospital admissions.

When hospital care is required, we ensure individuals are admitted only when essential and are supported to return home as quickly and safely as possible.

We also deliver a wide range of early intervention, reablement, and rehabilitation services, all focused on maximising independence and enabling people to remain in their own homes for as long as possible.

* Lead on safeguarding enquiries, applying experienced professional judgement to ensure adults are protected, risks are managed, and all statutory duties are met.
* Manage a demanding and varied caseload, prioritising work effectively while maintaining high standards of practice and compliance.
* Work as part of the Adult Social Care Hub, collaborating with multidisciplinary professionals to deliver timely, proportionate interventions and ensure people receive the right care at the right time.
* Build strong relationships with individuals, families, and community networks, enabling people to remain independent and connected within their own communities
* Use a strengths‑based approach to promote independence, reduce reliance on long‑term services, and improve outcomes.

About you...

You will bring at least two years of experience working within statutory social services, ideally in a local authority setting. You will demonstrate strong safeguarding practice, confidently manage a high and varied caseload, and apply up to date legislation and policy to deliver high quality outcomes. Collaborative working and sound professional judgement are essential.
