Job Title: Therapy Assistant Practitioner Frailty

Location: BGH

Department: AHP Services

Contract type: Permanent

Hours: 63.64

We have an exciting opportunity for highly motivated and innovative Therapy Assistant Practitioners to join our newly formed Frailty Unit in NHS Borders. You will have excellent communication and interpersonal skills to complete clinical and non-clinical duties together with service development activities.

You will work alongside a team of Occupational Therapists, Physiotherapists and wider MDT in the Frailty Unit, Emergency Department and the Medical Assessment Unit (MAU) to support prevention of admission and ensure timely, safe and appropriate discharge.

You will work independently with patients, within protocols and with guidance from a qualified Allied Health Professional (AHP), to assess and formulate an intervention plan to prevent an admission to hospital or facilitate a safe discharge. You will also undertake some administrative duties to help the effective running of the team. Thorough competencies have been developed to support induction.
