About the Role
In this role, you will support individuals with Autism, Learning Disabilities, Behaviours of Concern, Physical Disabilities, Mental Health conditions, Epilepsy and Complex Needs.
You will deliver high-quality, person-centred support aligned with our Building Better Lives (BBL) framework and the principles of Active Support, ensuring every individual is supported to live as an equal citizen.
As this is a new service opening, you will play a key role in creating a warm, safe and homely environment that meets CQC’s “Right support, right care, right culture” standards from the outset.
Your focus will be to provide reliable, personalised support that reflects each individual’s goals, wishes and aspirations, always respecting that you are supporting someone in their own home.
You may occasionally be asked to work across different services to ensure flexible, person-centred support.
Key Responsibilities
5. Promote independence, life skills and informed choice in line with individual support plans
6. Follow Positive Behaviour Plans and record incidents accurately
7. Support individuals to communicate their needs and preferences using appropriate communication approaches
8. Maintain dignity, privacy and confidentiality at all times in line with GDPR and organisational policy
9. Act as a keyworker where required, ensuring individuals’ voices are heard
10. Support people to maintain relationships and develop social networks
11. Enable access to community facilities, activities and meaningful opportunities
12. Complete medication training and administer medication following competency assessment
13. Support with health, wellbeing and personal care, including continence care if required
14. Provide emotional support during challenging or difficult times
15. Work consistently as part of a team to ensure high standards of care
16. Maintain accurate daily records and follow safeguarding procedures
You will help establish strong team values, consistency of practice and a positive culture within this brand new service.
Essential:
17. Willingness to complete all required training
18. Willingness to work towards a Level 2 Diploma in Health & Social Care
19. Good basic literacy, numeracy and communication skills
20. Basic IT skills
21. Caring, empathetic and person-centred values
22. Ability to work independently and as part of a team
23. Flexibility to work evenings, weekends and bank holidays
Desirable:
24. Level 2 Diploma in Health & Social Care (or working towards)
25. Experience supporting individuals with Autism, Learning Disabilities or Complex Needs
26. Experience assisting individuals with mobility or physical health needs
27. Full UK driving licence
