Specialist Medicine care group requires an enthusiastic, dynamic, and highly motivated care pathway facilitator to assist with discharges and flow across the care group. You will be required to carry out paperwork according to the pathways that the discharge falls under, plan discharges early from admission, and possess excellent communication skills. Confidence to challenge in difficult situations and cross-cover at KGH if needed are essential. The post is Monday to Friday currently but may change in the future. You will work as part of the MDT team and collaborate with external professionals to ensure the safest discharge for our patients.

This role involves joining the team of existing care pathway facilitators to plan and manage timely and appropriate transfers of care (discharge) for patients from the hospital. The focus is on supporting the Trust's goal to increase discharges before 10 am, particularly for patients who no longer meet the criteria to reside in an acute hospital bed, aiming for same-day discharge. The post holder will work closely with ward teams to identify potential discharge issues early and communicate effectively with patients and families/carers upon admission. Collaboration with multidisciplinary teams and system partners is crucial to ensure smooth and safe discharges, with plans well communicated to all parties.

Our main sites are KGH in Goodmayes and Queens Hospital in Romford. We operate two busy emergency departments serving over 330,000 visitors annually. We are campaigning for £35m to transform the A&E at Queens and eliminate corridor care. Our patients benefit from initiatives like the Women’s Health Hub in Ilford, the Ageing Well Centre in Hornchurch, and Community Diagnostic Centres at Barking Community Hospital and St Georges Health and Wellbeing Hub in Hornchurch. These CDCs operate 12 hours a day, 7 days a week, providing an additional 88,000 scans annually.
