Overview
As a Therapist within the Home First Team you will be involved in many aspects of a patient’s journey from hospital to home. The team supports patients within the Emergency Village (EV) where additional support is needed to return home; participates in triage of patient referrals with MDT discussion to determine the most suitable pathway; and works in patients’ homes or short-term placements to help achieve rehabilitation goals. This role is primarily based in the community, with flexibility in assessment locations as the service develops (hospital, home or the Emergency Village in Salford Royal).
Home First utilises a Strengths Based Approach in its assessment and intervention, and the Multi-Disciplinary Team (MDT) includes therapists, technicians, nurses, social workers, pharmacists, housing workers, reablement support workers, assistant practitioners and care coordinators. We work closely with patients, families and partner organisations and liaison with non-statutory services where identified and appropriate.
To support and facilitate timely hospital discharges by providing appropriate assessments and to support with the triage process, taking part in discussions around discharge destinations for patients in hospital or those in the community who may need a short period of rehabilitation and reablement. To assess and treat people in their own homes who are experiencing reduced physical and cognitive function with the goal of returning to independence after a functional deficit. To provide advice on the management of these patients to other Health and Social Care Professionals, including appropriate community placements. To support the wider team and assist in the development of services within the MDT environment, including leadership and supervision of staff and students as part of the essential criteria. To provide a therapy service to people in their own homes across Salford on a 7-day working pattern.

Responsibilities
* Provide occupational therapy assessment and treatment in patients’ homes or other agreed settings to support independence after functional decline.
* Participate in the triage process and lead or contribute to discussions about discharge destinations for in-hospital or community patients requiring short-term rehabilitation and reablement.
* Collaborate with the MDT to develop and deliver patient-centred care pathways and signpost to appropriate local services where identified.
* Support hospital discharge planning to ensure timely and appropriate discharges.
* Support and develop services within the MDT environment, including leadership and supervision of staff and students.
* Deliver therapy services across Salford on a 7-day working pattern.
Qualifications and experience
* Experience working within a multi-disciplinary team and ability to work as part of a team.
* Demonstrated leadership and supervisory experience with staff and students (essential criterion).
* Ability to assess and treat patients at home and to liaise with other health and social care professionals.
