The role is a key clinical role focused on delivering Physiotherapy and Case Management for patients with complex needs within community discharge hubs, interim health-funded beds, and inpatient setting on Chartridge Ward.
The postholder will provide holistic assessments, evidence-based treatment, and person-centred discharge planning to promote safe, timely transitions of care. Working under the supervision and guidance of Senior Physiotherapists, the role supports recovery, independence, and prevention of avoidable readmissions.
The postholder will also contribute to the ongoing development of clinical practice, service improvement, and multidisciplinary collaboration across the Trust’s discharge and pathways.
The role requires flexibility to work across a range of sites countywide as directed.
Main duties of the job
An exciting opportunity has arisen to join our Hub Bed Physiotherapy Service, supporting patients with complex needs across a variety of community sites.
Although this is not a rotational post, you will be well supported through a nominated Band 6/Senior mentor who will provide guidance, supervision, and ongoing development. You will work closely with Physiotherapy support staff, offering opportunities to develop your skills in delegation, supervision, and team leadership.
Patients within the Hub Bed Service often present with complex multi-pathologies, allowing you to develop a broad and varied skill set across elderly care, falls prevention, neurology (including stroke and Parkinson’s disease), respiratory care, and orthopaedics. The service operates across both community settings and a community hospital, providing valuable experience in both areas of practice.
In-service training and continuing professional development are actively encouraged and supported.
This is a county-wide role involving community-based work, including visits to patients in care homes and access visits. Applicants must therefore be able to travel independently for work purposes and hold a valid driving licence, with access to a vehicle.

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
10. Registered Allied Healthcare Professional with HCPC - Physiotherapist
11. Degree/ Diploma in Physiotherapy
Desirable criteria
12. MCSP Membership
EXPERIENCE
Essential criteria
13. Experience of working within the NHS
14. Experience of working within multiagency/multi-disciplinary team
15. Commitment to continuing professional development and evidence of a CPD portfolio
16. Knowledge and application of evidence based practice and clinical reasoning

SPECIAL CIRCUMSTANCES
