Overview
The Home First Team Manager is responsible for the effective day-to-day operation of the Home First service.
This includes leading a multidisciplinary team of assessors, community support workers, and administrative staff to ensure the safe and timely delivery of person-centred care that supports recovery and promotes independence at home.

Oversee the planning and coordination of staff rotas, ensuring appropriate deployment of support workers and assessors across the locality to meet service demands.
Monitor service delivery to ensure it aligns with Home First priorities and complies with CQC standards, organisational procedures, and national regulatory framework.
Hold responsibility for the management of a defined caseload, ensuring timely assessments, support planning, and discharge coordination.
Provide direct line management to Band 5 and Band 4 Home First Assessors, community-based support workers, and admin staff, ensuring they are well-supported, supervised, and developed in line with service expectations.
