About the Role:
We are seeking an experienced and motivated Specialist Social Worker to join our Adult Mental Health Service. This is an exciting opportunity to play a key role in delivering high-quality, person-centred social work services to adults, their families and carers.
The successful candidate will plan and provide a comprehensive range of specialist social work services, designing and implementing appropriate packages of care in accordance with Care Management principles and Humber Mental Health Foundation Trust systems. You will also lead on social care practice within the multidisciplinary team, supporting the development of high standards across the service.
Key Responsibilities:
* Provide a range of specialist assessments and interventions for adults, their families and carers, including reassessments and the design, implementation and review of care packages.
* Deliver specialist interventions as part of the care planning process, ensuring outcomes are regularly reviewed and adjusted where necessary.
* Work collaboratively with partner agencies to plan, arrange, coordinate and monitor services to meet identified needs.
* Arrange and chair multi-agency meetings as required.
* Assess and respond to mental health needs in line with policy, departmental procedures and professional standards.
* Maintain accurate case records and contribute to statistical returns and quality monitoring systems.
* Provide training and consultancy within your specialist area to departmental staff and external agencies.
* Represent the Department at local and regional multi-agency forums and specialist groups.
* Deputise for the Team Leader as required.
* Undertake additional duties consistent with the level and responsibilities of the post.
