Aberdeen City OOH Band 5 Community Nurse
Opportunities have arisen for Band 5 Community Nurses to join our Aberdeen City HSCP Evening and Night Community Nursing Unscheduled Care Team.
You will be part of a team that predominately provides person-centred, end-of-life care and symptom control for patients in their own homes, enabling them to remain in their preferred place at end of life. You will also deliver unscheduled clinical nursing interventions to patients referred from GMED, NHS24, Hospital@Home and other professionals.
Evidence of current post-registration experience in a clinical environment is essential. Community Nursing experience is preferred. Good communication skills are essential, as well as experience in palliative and end-of-life care.
Responsibilities
* Provide person-centred end-of-life care and symptom control for patients in their own homes, enabling them to remain in their preferred place at end of life.
* Deliver unscheduled clinical nursing interventions to patients referred from GMED, NHS24, Hospital@Home and other professionals.
* Work with the Aberdeen City HSCP Evening and Night Community Nursing Unscheduled Care Team.
Qualifications / Requirements
* Evidence of current post-registration experience in a clinical environment is essential; Community Nursing experience is preferred.
* Good communication skills and experience in palliative and end-of-life care.
* Car owner/driver and a current UK driving licence; car must be LEZ compliant.
* Hours: various hours available within an 8 day rota. 1900-0800 (3 shifts on / 5 off) pro rata 31.5 hrs per week; 1830-0000 (3 shifts on / 5 off) pro rata 14.5 hrs per week.
