Overview
We are recruiting a Clinical Lead to join the Ellesmere Port East Care Community Team. The role is part‑time, 25 hours per week, and involves providing clinical leadership within a multidisciplinary team.
Responsibilities
* Provide a highly visible clinical profile with leadership and oversight of a designated community caseload.
* Work collaboratively with the wider health, social care and third‑sector partner agencies to meet patients’ needs.
* Ensure patients receive the right care, by the right person at the right time and place.
* Ensure patient‑centred decisions, streamline care journeys and support independence and quality of care.
* Provide direction within the caseload, considering current health and social care agendas and local initiatives.
* Stay informed of local public‑health community profiles and their impact on care delivery.
* Understand the skills required from the Community Care Team according to community profiles.
* Focus on the delivery of person‑centred care.
* Provide effective, evidence‑based care to a defined population through caseload management and monitoring of care standards.
