Occupational Therapist - Community Resource Team; 37 hours per week.
Responsibilities
 * Provide the Occupational Therapy input to the Support at Home enabling home care service, with a focus on maintaining or improving the functioning of people living with dementia or cognitive impairment.
 * Deliver short-term, time-limited Occupational Therapy interventions as part of a team of Social Care Workers; provide professional guidance to the team.
 * Be part of an integrated health and social care service offering a range of short-term interventions to promote independence in people’s own homes.
 * Maintain a defined caseload and provide evidence-based interventions to promote independence and quality of life.
 * Plan and manage time efficiently to complete clinical and non-clinical tasks, ensuring effective workload management and ability to respond flexibly to service needs.
 * Utilise a broad range of Occupational Therapy skills while working with people with a wide range of disabilities, their carers and families.
 * Work in a community setting, often with individuals in their own homes, and collaborate with team members to achieve goals.
Qualifications & Requirements
 * Full driving licence and access to a vehicle is essential.
 * Excellent communication and organisational skills; ability to plan and manage time effectively.
 * Enthusiastic, imaginative, and able to problem-solve in a community setting.
 * Must be able to conduct a basic conversation through the medium of Welsh.
 * Protecting children, young people or adults at risk is a core responsibility of all council employees.
 * Enhanced DBS check (Adults) is a requirement for this post.
 * A valid driving licence is a requirement for this post.
