We are seeking a dedicated and compassionate Part-Time Care Assessor to join our team. In this role, you will be responsible for assessing the needs of individuals requiring care and support. Your expertise will help ensure that each person receives tailored assistance that enhances their quality of life. The ideal candidate will possess strong teaching skills, enabling them to effectively communicate and educate clients and their families about care options.
Responsibilities
* Conduct thorough assessments of clients' care needs in a respectful and empathetic manner.
* Develop individualised care plans based on assessment findings and client preferences.
* Collaborate with healthcare professionals, families, and caregivers to ensure comprehensive support for clients.
* Provide guidance and education to clients and their families regarding available resources and care options.
* Maintain accurate documentation of assessments, care plans, and any changes in client needs.
* Stay informed about best practices in care assessment and relevant legislation to ensure compliance.
Requirements:
* Level 3 in Health and Social Care (or equivalent qualification) – essential
* Full UK driving licence and access to a vehicle – essential
* Good communication skills and proficiency in English – essential
* Previous experience in home care or care assessments is desirable
* Strong organisational and people skills
