Qualified Adults Social Worker Merseyside | £32 per hour (Ltd) | Full-time
Make a real impact supporting adults during times of change and vulnerability. We're looking for an experienced and passionate Qualified Social Worker to join a dedicated and dynamic Adult Social Care Team in Merseyside. This is a peripatetic, hospital-based role supporting adults through safe discharges, Care Act assessments, and complex safeguarding cases. You'll work across two hospital sites, holding cases and helping individuals navigate crucial stages in their care.
Your Role Will Involve:
1. Conducting Care Act 2014 assessments and developing person-centred care plans
2. Leading on safeguarding investigations and managing complex cases
3. Working with multi-disciplinary teams to support timely and safe hospital discharges
4. Applying relevant legislation including the Mental Capacity Act, Mental Health Act, and Human Rights Act
5. Managing a varied caseload and participating in duty as required
What We’re Looking For:
6. Social Work England registered
7. Qualified Social Worker, with a degree or equivalent
8. Minimum 3 years' post-qualified experience in Adult Social Care
9. Strong understanding of adult safeguarding, risk management, and hospital discharge planning
10. Confident communicator with excellent assessment, negotiation, and report-writing skills
11. Able to build effective relationships with service users and partner agencies
12. Passionate about delivering high-quality, person-centred care and support
Location & Accessibility: This role is easily commutable from Liverpool, Wirral, Widnes, Warrington, Huyton, Wallasey, Wigan, St Helens, Chester, Skelmersdale, and Preston.
