Stubbington Medical Practice are looking for an experiencedAdvanced Nurse Practitioner to work as an active member of the same day urgentcare duty team.


Main duties of the job

As such you would be required to:

Remotely triage patients by telephone and/or face to face if clinically indicated. Diagnose, treat, and refer where necessary.

Be an Independent Nurse Prescriber with experience in Primary Care and hold an MSc Post Graduate Diploma or equivalent in minor illness.


About us

Stubbington Medical Practice is situated in the small village of Stubbington, close to the sea and has approximately 12700 patients with ahigher-than-average elderly population.

Our clinical team comprises of 7 partners, 2 salaried GPs, 2registrars, 2 ANP's (1 Lead of nursing team), 3 nurse practitioners, 1 practicenurse and 4 HCAs, supported by our Coastal PCN Home Visiting Team.

We are a friendly practice and support each other to offer outstanding care to our patients.

JobSummary

The postholder is an experienced Advanced Nurse Practitioner who, acting within their professionalboundaries, will provide care for the presenting patient from initial history taking,clinical assessment, diagnosis, treatment and evaluation of care. They willdemonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the general practice. Thepost holder will demonstrate critical thinking in the clinical decision-makingprocess. They will work collaboratively with the general practice team to meetthe needs of patients, supporting the delivery of policy and procedures, andproviding nurse leadership as required. In order to work at this level NMCrequirements for advanced practice must be met.

Keyresponsibilities

ClinicalPractice

The Advanced Nurse Practitioner works autonomously and isaccountable for own professional actions.

Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnosis

Clinically examine and assess patient needs from a physiologicaland psychological perspective, and plan clinical care accordingly

Assess, diagnose, plan, implement and evaluateinterventions/treatments for patients with complex needs. Proactively identify,diagnose and manage treatment plans for patients at risk of developing along-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions,integrating both drug- and non-drug-based treatment methods into a managementplan

Prescribe and review medication for therapeutic effectiveness,appropriate to patient needs and in

accordance with evidence-based practice and national and practiceprotocols, and within scope of practice

Work with patients in order to support compliance with andadherence to prescribed treatments. Provide information and advice onprescribed or over-the-counter medication on medication regimens,side-effects interactions

Prioritise health problems and intervene appropriately to assistthe patient in complex, urgent or emergency situations, including initiation ofeffective emergency care

Support patients to adopt health promotion strategies thatpromote healthy lifestyles, and apply principles of self-care

Communication

Utilise and demonstrate sensitive communication styles, toensure patients are fully informed and consent to treatment

Communicate effectively with patients and carers, recognisingthe need for alternative methods of

communication to overcome different levels of understanding,cultural background and preferred ways of communicating

Anticipate barriers to communication and take action to improvecommunication

Maintain effective communication within the practice environmentand with external stakeholders

Act as an advocate for patients and colleagues

Ensure awareness of sources of support and guidance (eg PALS)and provide information in an acceptable format to all patients, recognisingany difficulties and referring where appropriate

Delivering a quality service

Recognise and work within own competence and professional codeof conduct as regulated by the NMC. Produce accurate, contemporaneous andcomplete records of patient consultation, consistent with legislation, policiesand procedures

Prioritise, organise and manage own workload in a manner thatmaintains and promotes quality

Deliver care according to NICE guidelines and evidence-basedcare

Assess effectiveness of care delivery through self and peerreview, benchmarking and formal evaluation

Initiate and participate in the maintenance of qualitygovernance systems and processes across theorganisation and its activities

Utilise the audit cycle as a means of evaluating the quality ofthe work of self and the team, implementing improvements where required

In partnership with other clinical teams, collaborate onimproving the quality of health care responding to local and national policiesand initiatives as appropriate

Evaluate patients response to health care provision and theeffectiveness of care

Support and participate in shared learning across the practiceand wider organisation

Use a structured framework (eg root-cause analysis) to manage,review and identify learning from patient complaints, clinical incidents andnear-miss events

Assess the impact of policy implementation on care delivery

Understand and apply legal issues that support theidentification of vulnerable and abused children and adults, and be aware ofstatutory child/vulnerable patients health procedures and local guidance

Leadership personal and people development

Encourage and motivate others to be forward thinking in ideas thataddress the health needs of the practice that will be innovative whilstmanaging and developing new ways of working,

Take responsibility for own learning and performance includingparticipating in clinical supervision and acting as a positive role model

Support staff development in order to maximise potential in thementorship and assessment of trainee Nurse Practitioners/Practice Nurses andundertake mentorship for more junior staff, assessing competence against setstandards

Critically evaluate and review innovations and developments thatare relevant to the area of work

Contribute to the development of local guidelines, protocols andstandards

Market the role of the advanced nurse practitioner in generalpractice

Understand own role and scope and identify how this may developover time by assessing own learning needs and undertake learning as appropriate

Work as an effective and responsible team member, supportingothers and exploring the mechanisms to develop new ways of working

Delegate clearly and appropriately, adopting the principles ofsafe practice and assessment of competence

Create clear referral mechanisms to meet patient need

Prioritise own workload and ensure effective time-managementstrategies are embedded within the culture of the team

Work effectively with others to clearly define values, directionand policies impacting upon care delivery

Discuss, highlight and work with the team to createopportunities to improve patient care by providing an educational role topatients, carers, families and colleagues in an environment that facilitateslearning

Agree plans and outcomes by which to measure success

Management of risk

Manage and assess risk within the areas of responsibility,ensuring adequate measures are in place to protect staff and patients

Monitor work areas and practices to ensure they are safe andfree from hazards and conform to health, safety and security legislation,policies, procedures and guidelines

Apply infection-control measures within the practice accordingto local and national guidelines

Advocate for policies that reduce environmental health risks,are culturally sensitive and increase access to health care for all

Interpret national strategies and policies into localimplementation strategies that are aligned to the values and culture of generalpractice

Managing information

Use technology and appropriate software as an aid to managementin planning, implementation and monitoring of care, presenting andcommunicating information

Review and process data using accurate Read codes in order toensure easy and accurate information retrieval for monitoring and auditprocesses

Manage information searches using the internet and local librarydatabases

Understand responsibility of self and others to the practice andprimary care trust regarding the Freedom and Information Act

Learning and development

Undertake mentorship for more junior staff, assessing competenceagainst set standards

Disseminate learning and information gained to other teammembers in order to share good practice and inform others about current andfuture developments (eg courses and conferences)

Assess own learning needs and undertake learning as appropriate

Provide an educational role to patients, carers, families andcolleagues in an environment that facilitates learning
