We are recruiting for an Advanced Clinical Practitioner to support generalpractice in Walsall and in particular, the PCN and its objectives. The successful candidate will work as an autonomous practitioner working independentlyand with other health care professionals to assess, diagnose and treat patientswith a range of acute, non-acute and chronic medical conditions, initiatingtreatment as appropriate. The post includes working within the Walsallpractices.
The post will work with all the 5 clusters of GP surgeries known as PrimaryCare Networks across the North, East, South or West of Walsall. The post willbe based at and provide the service from the GP surgeries, with somerequirement to attend team meetings and training at OurNets Head Office,located in Bloxwich, as well as training at other local or regional venues asnecessary.
ACPs:
Candidates will have a masters degree. Is working at a masters levelaware or equivalent that encompasses the four pillars of clinical practice,leadership and management, education and research, with demonstration of corecapabilities and area specific clinical competencies. Has qualified from anapproved 18-month training pathway or equivalent and qualified as anindependent prescriber
Main duties of the job

To trial and adopt new working methodsdesigned to manage the increasing demand in primary care.
To provide education and training toother staff and students as appropriate and to take a lead role in encouragingteamwork and sharing best practice.
To undertake audit and research if andwhen required.
To promote healthy lifestyles, healthpromotion and disease prevention, by suggesting and recommending possiblesolutions.
Manage caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence

Ensurethat patients are referred to the appropriate healthcare professional for theappropriate level of care within an appropriate period of time eg. Pathologyresults, common/minor ailments, acute conditions, long term condition reviews,etc.

Qualifications
* Qualifications and Registrations
* Reguistered Nurse (NMC) or Allied Health Professional with current registration (HCPC)
* Masters - Advanced Clinical Practice (or working towards with substantial ACP experience)
* Understanding of national and local guidelines
* Primary Care Experience
* Experience using EMIS
* Knowledge of local care pathways and community services.
