The Foster Care Coordinator uses trauma-informed, evidence-based approaches with members and caregivers for children and youth in out-of-home foster care, especially those with complex or moderate needs, not enrolled in a waiver, on a waitlist, or receiving services through a CCBHC.
This is a field-based role located in Reno, Butler, Sedgwick Counties, KS, and surrounding areas. Kansas residents may telecommute*.
Primary Responsibilities:
1. Assess, plan, and implement individualized care strategies considering permanency needs and least restrictive care levels.
2. Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services.
3. Manage care plans as the single point of contact throughout the care continuum.
4. Communicate health-related information to stakeholders for coordinated care.
5. Advocate for children, youth, and families to ensure their needs and choices are supported.
6. Provide trauma-informed and specific education to members and caregivers.
7. Coordinate with child welfare and juvenile justice systems.
8. Coordinate additional services like educational advocacy, behavioral health, and early childhood interventions.
9. Complex cases will have oversight by master’s level case managers, including review of assessments and care plans.

Required Qualifications:
1. Master’s Degree in Social Work, Nursing, Psychology, or related human services field, or
2. Unrestricted, independent licensure as a Registered Nurse, or
3. At least 2 years of community health experience, 1 year working with multi-system children, and proficiency with MS Word, Excel, Outlook.
4. Ability to travel within the region and reside in Reno, Butler, or Sedgwick Counties, KS, with reliable transportation and a valid US driver’s license.
Preferred Qualifications:
1. Licensed Social Worker or clinical degree.
2. Experience managing populations with complex needs.
3. Experience with electronic charting and community resource coordination.

The salary range is $28.61 to $56.06 per hour, based on full-time employment and various factors including location, education, experience, and certifications. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions.
