Liquid Personnel is recruiting a General Occupational Therapist to join their Adult Social Care OT service for its client. This is a broad community-based role, suitable for an Occupational Therapist who is confident in undertaking functional assessments, recommending equipment and adaptations, and working independently across the borough.
You will support adults with a wide range of physical, cognitive, and social needs, enabling safer and more independent living within their homes and communities.
Working Pattern: Hybrid / community-based (as required by the service)
What will your responsibilities be?
1. Complete holistic functional assessments in homes and community settings.
2. Recommend and arrange equipment and minor adaptations to support safety and independence.
3. Apply strengths‑based, person‑centred approaches in all assessments.
4. Develop and review moving & handling plans, ensuring safe techniques and proportionate recommendations.
5. Work closely with social workers, housing teams, health colleagues, families, and carers.
6. Maintain clear, accurate, and timely case records in line with service standards.
7. Contribute to prevention‑focused practice, early intervention, and effective risk management.
8. Support timely discharge planning and community reablement pathways where required.
Qualifications & Experience:
To be successful in this role you must have,
9. Degree / Diploma in Occupational Therapy.
10. Experience working in adult social care, community OT, reablement, or similar settings.
11. Confident with equipment provision, risk assessments, and clinical decision‑making.
12. Strong communication and problem‑solving skills.
13. Ability to manage an independent caseload in a busy environment.
