You have experience working with older adults or adults with physical disabilities, either in the community or a hospital setting. You possess a strong understanding of the needs of the elderly and disabled, are committed to equality of opportunity, and are eager to engage in continuous training and development. What the job involves The Social Work Assistant will be based at County Hall within the Adult Services Review Team. You will collaborate with various team members including Social Workers and Duty Workers, under the direction of a Team Manager. Your responsibilities include arranging and monitoring services according to individual care plans, providing advice, liaising with agencies, completing care plan reviews, and participating in team meetings. Additionally, compliance with health and safety legislation and Council policies is required, along with readiness to work from various locations as needed.
