Role 1
Front Door Team - To provide an outcome focussed assessment service to all adults with physical disabilities or age related problems.
Role 2
Hospital Team -
* To provide an outcome focussed assessment service to all adults with physical disabilities or age related problems.
* Local Authority and hospital discharge experience required.
* Knowledge of the Mosaic case management system is desirable
* Hybrid set up, min 2-3 days a week in the office
* Must be a driver with a valid driving license and a vehicle to use for work
