We are seeking a dedicated and experienced Qualified Social Worker to join our Adult's Social Work Assessment Team in Liverpool. This is an exciting opportunity for a passionate individual who is committed to improving the lives of adults in our community. The successful candidate will play a crucial role in conducting assessments, developing care plans, and ensuring the delivery of high-quality support services.
Responsibilities
* Conducting comprehensive assessments of adults to identify their needs and strengths.
* Developing and implementing care plans in collaboration with individuals and their families.
* Working closely with multidisciplinary teams to ensure coordinated service delivery.
* Monitoring and reviewing care plans to ensure ongoing effectiveness and compliance with statutory requirements.
* Providing expert advice and guidance to colleagues and stakeholders on adult social care issues.
Qualifications
* A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
* Registration with Social Work England.
* Proven experience in adult social work, particularly in assessment and care planning.
* Strong knowledge of relevant legislation, policies, and best practices in adult social care.
* Excellent communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
