Health and Therapy Assistant Home First Chichester

We're bringing together nurses, therapists, health and therapy assistants to work alongside acute multi-disciplinary teams and transfer of care hubs to promote an efficient, effective hospital-to-home transition for our patients.
Our multi-disciplinary Home First teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm.
Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission.
Home First will, where required, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

Main duties of the job
As a Health and Therapy Assistant you will be providing personal, health and therapy care for people across Sussex with acute and diverse needs.Your day will be varied and interesting, supporting people to achieve greater independence in their daily lives.
You will assist multi-disciplinary colleagues in the ongoing assessment and provisionof care and reablement as prescribed within the personalised care plan. This includes simple clinical and therapeutic procedures such as taking physical observations, venepuncture, administeringmedications, simple wound care, rehabilitation programmes and equipmentprovision. You will observe changes in peoples' status and keep your team informed through verbal updates and accurate record keeping.
You don't need previous experience; we'll train you on the job, supporting you to be the best that you can be. Your training as a Health and Therapy Assistant will include basic nursing and therapy skills and we'll support you to achieve the Care Certificate. Through our comprehensive competency framework we will support you to achieve continuous development.
You will need to able to drive with a full UK licence and access to a vehicle due to the geographic area covered by the team.

Job responsibilities
With experience and further training, you could further your career in health care opening opportunities to train as aNursing Associate. Person Specification
Qualifications
* General standard of education to GSCE level or equivalent
* NVQ level 2 or equivalent (in health and social care applicable subject), or be willing to undertake
* Care Certificate
Experience
* Evidence of consistent pattern of learning from education, training and experience
* Experience of working in a health or social care setting
* Experience of working as a care assistant either with nursing or therapy services
Skills
* Able to use recognised IT packages e.g. Microsoft Office including Excel
* Able to use of IT systems and keyboard skills including accurate data recording skills
* Flexible approach to working
Other
* Car driver (Valid Driving License for use in the UK)
* Access to a vehicle for work purposes
* Willingness to undertake training to develop within the role
