This position is based within the Hospital Discharge Team, working closely with partner agencies to ensure assessments are completed in a timely manner to facilitate the flow out of hospital sites. Experience in this area is preferable but not essential; applications are welcome from Social Workers, Occupational Therapists, and Nurses.
Responsibilities
* Provide a statutory Social Work Service in accordance with relevant legislation and guidelines.
* Carry out assessments of service users' needs and risks, using accredited risk assessment tools when necessary.
* Deliver direct interventions to individuals and small groups as part of a structured program.
* Participate in supervision sessions and represent the Social Work Service in multidisciplinary forums.
