Summary
We are seeking a Full time (Fixed Term 12mths) Integration and Outreach Manager to oversee the day to day management of the Commnunity Health & Wellbeing Workers. This pivotal role will enhance our outreach efforts, strengthen community relationships, and promote our healthcare services. The Outreach Manager will play a crucial part in advancing our mission of addressing health inequalities and delivering quality patient care.
Responsibilities
* Identify and Develop opportunities for enhanced integration across Integrated neighbourhood Teams
* Work in partnership with Primary Care Networks (PCNs), local authorities, and voluntary sector organisations to improve communication and service coordination.
* Lead initiatives to improve community engagement and outreach, to ensure equitable access to health, care and wellbeing services.
* Lead on VCSE engagement for the CHWW pilot, both at Place level and in hyperlocal areas of the pilot.
* Coordinate thematic insights & opportunities for systematic change sharing, though Communities of Practice, Deep End Network and INT. As well as wider Bucks partnerships including Community Boards and Opportunity Bucks programmes
* Deputise for the Integration Leads as required, representing the programme in strategic meetings and decision-making processes.
* Lead on the development of communications strategies to promote the work of CHWWs pilot.
* Monitor service performance and workforce metrics, providing regular reports to the Integration Lead, INT programmes and other reporting and governance structures.
* Skills
* Proven experience in leadership or management roles within health /social care or community services.
* Strong understanding of Integrated Neighbourhood Teams (INTs), Primary Care Networks (PCNs), Secondary Health Care, Local Authority and the Voluntary Community Social Enterprise (VCSE) sector.
* Knowledge of Inclusion Health Groups and health inequalities.
* Ability to engage with diverse communities and build trusted relationships.
* Strong communication skills, including the ability to develop and deliver outreach strategies
* Experience in workforce management, supervision, and professional development.
* Data literacy and experience using digital tools for service monitoring and impact measurement.
* Ability to deputise for senior leaders and make informed strategic decisions.

This role plays a pivotal part in strengthening integration, addressing health inequalities, and ensuring effective community health service delivery.
The role may require flexible working, including some travel across INT areas, attendance at regional meetings or rare evening/weekend work.
We are unable to offer this role access to the NHS pension, but an Aviva scheme is available.
