NHS Integration Coordinator

The Growth Company’s (GC) Employment, Justice and Health team have an exciting new opportunity for an NHS Integration Coordinator to raise the profile of the contract and GC Company and contributing to referrals and establishing co location with key stakeholders.

In Employment, Justice and Health, we deliver integrated, person-centred support to help individuals with complex barriers move towards and into work. We support people affected by health conditions, justice involvement and long-term unemployment, working to improve wellbeing, confidence and employability while connecting them with inclusive job opportunities through employer partnerships.

This role will be based in the community, covering Trafford Park and Salford. Therefore, a full UK driving licence and access to a personal car are required.

Key Responsibilities:

1. Build, broker and maintain strong relationships with NHS primary care teams, mental health services, social prescribing networks and Voluntary and Community Sector (VCS) organisations to generate a consistent pipeline of participant referrals.
2. Work collaboratively with key stakeholders to coordinate and secure co‑location opportunities, improving accessibility for participants and strengthening integration between services.
3. Deliver engaging presentations, workshops and outreach activities across partner and community settings to raise awareness of the programme, increase engagement and drive referrals.
4. Plan and deliver targeted events and project activities to reach potential participant groups and expand referral pathways.
5. Accurately record all stakeholder engagement and outreach activity on internal systems to ensure robust reporting and performance tracking.

About You:

6. A confident, approachable and empathetic communicator who can quickly build trust with a wide range of stakeholders, from senior NHS leaders to community‑based organisations.
7. Naturally positive and people‑focused, with the ability to motivate, encourage and support individuals who may face multiple barriers to employment.
8. Highly organised and proactive, able to manage a busy workload, adapt to changing priorities and maintain momentum towards targets without compromising quality.
9. Someone who thrives in partnership‑driven environments and enjoys representing a programme with professionalism, enthusiasm and credibility.

Skills Required:

10. Strong experience of partnership working within the primary health care or community sector, including engaging senior stakeholders and navigating complex organisational structures.
11. Excellent communication and presentation skills, with the ability to tailor messages to different audiences and deliver sessions that are clear, engaging and impactful.
12. Confident using digital systems, databases, websites and social media, with the ability to maintain accurate records and produce reliable management information.
13. Proven project‑management capability, including planning and delivering events, coordinating outreach activity and managing timelines effectively.
14. Ability to work under pressure, meet referral and engagement targets, and maintain a high standard of service delivery in a fast‑paced environment.

We are currently unable to provide sponsorship for work permits or visas. Therefore, we can only consider candidates who already have the legal right to work in the UK.
