Personalisation Practitioner
We are looking for practitioners who can engage creatively with people with disabilities on a 1:1 basis, supporting them to identify opportunities and activities that reflect their interests, promote independence, and lead to meaningful outcomes. Responsibilities (not exclusive) are to:
* Build relationships with the individual/s and their families to support them and their son/daughter
* to participate in the planning, preparation and delivery of person centred/outcome focused leisure activities in a variety of settings or on occasion, in the individuals' home.
* To brief and de-brief parents/carers before and after events and to complete all necessary forms or reports.
* to work collaboratively with staff at PLUS and from other organisations.
* Implement the individual's plan to achieve stated outcomes
* Support young people with personal care such as toileting and feeding
* Promote independence
* Complete paperwork promptly (mostly electronic)
Qualifications and Skills
* Previous experience working with people with disabilities
* Excellent communication skills and the ability to build a rapport
* Good basic education
* Please note: you will be required to become registered with SSSC and undertake relevant qualifications to enable you in your role
* Highly Desirable: Driving license – with access to your own car
* Personal qualities willing to work flexibly, a belief in inclusion
* Strength in communicating

Job Type: Full-time, Part-time with hours ranging from 20-30 per week, this will include weekends
Salary: £12.60-£12.75 per hour
Contract length: we are looking for commitment for a minimum of 12 months
