Fareham and Portchester Primary Care Network - PCN is looking for amotivated Paramedic, Advanced Nurse Practitioner - ANP, Advanced ClinicalPractitioner - ACP, or an experienced Nurse working towards a Level 7qualification to join our dynamic and expanding same-day urgent care team andhome visiting team. This role is integral to our commitment to deliveringhigh-quality urgent care across four practices, focusing on acutely unwellpatients, as well as offering a unique opportunity to work a network of bothmedical and non-medical professionals to deliver Coordinated, holistic care.


Main duties of the job

Key Responsibilities

PatientCareProvide urgent care through telephonetriaging, in-practice consultations, and home visits.

Independentand Collaborative WorkAbility to work autonomously during home visits and collaboratively withina practice team.

TeamDevelopmentParticipate in and contribute totraining, mentoring, and peer support initiatives to promote teamdevelopment.

CollaborationLink withdoctors and other professionals during triage, home visits, andmulti-disciplinary team (MDT) meetings to deliver patient-centred care.

Holistic ApproachEngagein proactive care by combining medical opinions, medication reviews,mobility assessments, and health connections, with a focus on continuityand follow-up.

Integrated Patient CareWork closely with a multidisciplinary team to coordinate and execute shortand long-term care plans, ensuring patients receive comprehensive support.

Follow-upEnsure ongoingpatient support by following up on referrals and care episodes, monitoringprogress, and adjusting care plans as necessary.

Job responsibilities

Provide patient centred care

Monitor, review and evaluate careinterventions to ensure optimum quality is maintained

Inform clinical manager of any concernsregarding patients, relatives, visitors or staff that may compromise patientscare

Develop and maintain constructive workingrelationships and liaise effectively with all members of the multidisciplinaryteam so that patients needs are met

Communicate effectively with colleagues,patients and carers so that information is shared in order to meet patients needs in accordance with data sharing protocols

Act as a role model in the promotion ofperson centred practice, and challenge practice which is not person-centred, sothat a person-centred culture is maintained

Keep updated with relevant clinicaldevelopments and use knowledge to enhance standards of care

Practice, role model and promote safe andeffective extended skills in all area of clinical practice

Practice, role model and promote safe andeffective recording in line with policies and professional standard

Demonstrate care, compassion, competence,effective communication, courage and commitment with all patients and carers

Ensure patients with palliative care needs andtheir families have those needs met

Demonstrate competence and confidence inclinical practice, this includes all clinical procedures that arerelevant specialist to the area

Recognises own limitations in the provision ofclinical care and urgency of patients needs, referring to other healthcareprofessionals accordingly and is accountable for own actions

Promote an open reflective learning culture

Actively support other teams in the PCN whenneeded so that those teams can deliver high standards of care

Develop own professional skills and ensurethese are shared with the team and wider division, acting as a specialistresource

Use reflective practice to analyse incidentsand events and events to develop high standards of care

Ensure practice follows evidence basedpractice and national guidelines are followed

Ensure local prescribing guidelines arefollowed particularly in relation to the prescribing of antibiotics anddressings

Participates in setting standards andimplementing other implementing other quality initiatives

Participated on working parties and groups ata local and organisational level to aid in the development of professionalpractice this will include embracing innovations and proposing changes topractice

Ensurethat medicines in your area are handled and administered according toprofessional and organisational guidelines to ensure safety and efficiency

Person Specification


Qualifications

* Degree or equivalent in clinical work such as Advanced nursing or paramedic. Paramedics to have 3 year post qualification and 2 years consolidated learning
* Able to undertake clinical assessment of a patient having completed relevant course.
* Experience in Minor illness triage.
* Ability to work as an integrated team member
* Participation in CPD, appraisal and clinical governance
* Self-motivated
* Registration with Professional body - NMC or HCPC
* Safeguarding Level 2 and 3 Training
* Primary Care experience
* Work with EMIS Web clinical system
* Ability to prescribe autonomously, or working towards appropriate qualifications
* Previous home visiting primary care experience
* Independent and supplementary Nurse or paramedic prescribing course
