We currently have the following opportunity: Reablement Assistants (HART), 10 Posts – PKC13287 £27,836 – £30,295 Crieff Area Housing Office Are you ready for a career change that’s fulfilling and rewarding? If so, this is your chance to join our Health and Social Care Partnership as a Reablement Assistant, within Home Assessment Recovery Teams (HART), with opportunity to work in the North, and South localities of Perth and Kinross. This role is dedicated to working in our HART Team. You will work across the North / South locality of Perth and Kinross, this includes areas such as North – Blairgowrie, Alyth, Coupar Angus, Luncarty, Stanley, Bankfoot, Dunkeld, Pitlochry and Aberfeldy, and surrounding areas. South – Carse of Gowrie, Kinross, Auchterarder, Crieff, Comrie, and surrounding areas Does caring come naturally to you? Our team provide supportive care to vulnerable individuals across Perth and Kinross to allow them to remain living independently in their own homes for longer. It is essential that you have excellent listening, oral and written communication skills as well as the ability to work as part of a team. You must also have the ability to maintain confidentiality, work co-operatively with other professionals, families and carers. The Role, Responsibilities & Requirements The Role You will have the opportunity to visit and support people in their own homes, by following a pre-planned schedule and working on a shift pattern, that covers breakfast, lunch, tea and bed visits. By ensuring you support delivery of a quality care provision, this will support discharge from hospital, and help vulnerable individuals stay at home independently, and prevent admission to hospital or full-time care. You will support people with varying abilities and your responsibilities will depend on the needs of the individuals, this will include the promotion of reablement and independence while assisting with daily living tasks such as helping people to get up in the morning and ready for the day ahead. This can include personal care tasks whilst maintaining their dignity. You may also help with the preparation of meals, household tasks and practical day to day tasks to ensure physical well-being and maintain independence. You will support with administering and monitoring the taking of medication. At the end of the day, help them get ready for bed. Your responsibilities will include: Following a schedule / rota of individuals planned visits. Supporting individuals with personal care tasks to enable them to reach their optimum level of independence. Protecting individuals from risk or abuse and to report any concerns to your line manager. Ensuring Health & Safety for yourself and individuals, in line with PKC policy and procedures. Participate in risk assessments. Maintaining accurate records and report on individuals progress. Requirements You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross and demands related to service delivery. No entry requirement, but relevant experience is desirable. Required and supported to work towards SVQ in Social Services and Healthcare at SCQF level 6 (was SVQ2).
