About the Role
As a District Nurse you will carry a caseload of complex patients and contribute to the management of the teams.
Responsibilities
* Carry a caseload of complex patients.
* Manage team activities and contribute to management of the teams.
* Update or populate rosters and manage any sickness for the day.
* Support the wider workforce and complete any projects required.
* Work within a multi‑disciplinary integrated team to meet long‑term conditions and end‑of‑life care agenda, linking with GP services.
Qualifications & Skills
* Band 6 District Nurse qualification or just completed District Nurse Specialist Practice Qualification.
* Sound understanding of the current Nursing and Health agenda.
* Excellent communication, leadership and decision‑making skills.
* Ability to work autonomously while fostering a positive and supportive learning environment for team members and students.
* Commitment to the continuous development and modernization of community nursing.
Training & Development
The post is suitable for a newly qualified District Nurse, who will gain caseload management skills over a 6‑month preceptorship period.
Role Context
The post is part of the integrated Health and Social Care teams within the community setting in North Lincolnshire.
