Job Overview
Part‑time (18 hours per week) fixed‑term (6 months) Care Coordinator for first episode psychosis, based in South and West CMHT, Bradford. Band 6, salary £38,682‑£46,580 per annum.
Responsibilities
* Assess, plan and provide packages of care for individuals and families/carers experiencing first episode psychosis.
* Work in partnership with service users, carers and other partner agencies.
* Provide supervision and support to junior team members and colleagues.
* Maintain caseload at manageable levels, meet regular travel across the district.
Qualifications
* Professional qualification in mental health nursing, social work or occupational therapy.
* Experience of working as part of a multidisciplinary team.
Experience
* Care coordination under the Care Programme Approach.
* Substantial experience of care coordination in a mental health service (e.g. RGN).
* Working with adults, young people and children with serious mental health problems.
Desirable Experience
* Working with people who misuse substances.
* Working with families.
* Working in an Early Intervention Service / specialist psychosis service.
Knowledge
* Early Intervention in Psychosis.
* Understanding of the population within the Bradford and Airedale area and its specific needs.
