Job overview

An exciting opportunity has arisen for a Band 6 nurse to join our Older Adult CMHT.

Working alongside the wider MDT you will be responsible as the Care Coordinator of a caseload. This will include completing initial assessments and coordination of care needs, planning and implementation of evidence based care to a defined group of older adult service users and their carers within a defined catchment area/locality, and via a job plan.

You will be responsible to provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Hub Manager in their absence as appropriate.

You will act as a role model and resource for junior/less experienced staff.

You will also undertake duty work as allocated.


Main duties of the job

The post holder:

* Will act as an independent practioner within a defined community setting, while liaisingwith other professionals as necessary
* Will manage a defined caseload and provide supervision to others as appropriate.
* Will be responsible for the assessment planning implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
* Will be responsible for ensuring effective communication pathways exist.
* Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.
* Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.
* Participate as an active member of the duty system including the undertaking of emergency assessments

As part of the selection process you may be required to undergo a Disclosure and Barring Service (DBS) check, however all cases of criminal record history are dealt with on a case by case basis.

Please note, if applicable for your role you will be required to cover the cost of your DBS certificate. This will be deducted from your salary over 3 months. You will also be required to sign up to the DBS Update Service before commencement with the Trust. The Trust will reimburse this. You will be contractually obliged to maintain your subscription.

All successful applicants will be required to complete the Trust’s induction programme. The Fundamental (Statutory and Mandatory) training will be in an e-learning format. You will be required to complete training modules online (approx. 9-14 hours depending on post) by the end of your second week with us.
