Our services help people avoid hospital or accelerate them safely through it, with specialist support in their community or homes whenever needed. This role will support the delivery of initial assessments and booking calls for the NHS Diabetes Prevention Programme (NDPP), as well as promote both the NDPP and the Type 2 Diabetes Path to Remission Programme at engagement events across the local area. We are looking for a motivated and personable individual who can build and maintain strong relationships with key stakeholders, including primary care networks, community organisations, and local businesses. You will play a key role in increasing awareness of our services, encouraging referrals, and positioning the Health & Wellbeing team as a trusted partner within the health system. This will involve delivering presentations, attending community events, and representing the service in a professional and engaging manner. The role will also require working directly with service users via telephone to complete one-to-one initial assessments and booking calls as part of our central health coaching team. About the Role In this role, you will: Attend GP practices, local health groups, and primary care meetings to promote the services available in the area. Provide feedback following events and share new contacts with our central engagement team. Support communication and outreach activities (e.g., public events, training sessions, awareness campaigns, communications materials, social media) to promote the services and encourage referrals. Complete initial assessments and booking calls for the Diabetes Prevention Programme. What You’ll Bring We don’t expect perfection. Relevant health and wellbeing qualification such as (but not limited to) an undergraduate degree, diploma, or equivalent in Nutrition, Dietetics, Health Psychology, Sport Science and/or a CIMSPA fitness qualification at level 3 or above Experience in community engagement and partnership working.
