Post Holder Responsibilities:

* Provide clinical assessment and direct patient management.
* Make autonomous decisions involving complex facts and situations, analyzing and interpreting options for patient care.
* Identify needs for change, lead service development, and manage change, including negotiating and influencing outcomes.
* Conduct complex patient reviews, providing feedback on physical and mental health to inform care decisions, which may involve differing opinions.
* Lead and chair ward reviews, initiating and adjusting treatment plans.
* Prepare medical reports.
* Support doctor development by arranging clinical opportunities in other areas.
* Collaborate with Responsible Clinicians on patient detention and least restrictive principles.
* Use advanced clinical skills and knowledge for diagnoses and developing management plans for optimal care.
* Provide clinical leadership within the Advanced Clinical Practitioner Strategic Service.
* Be responsible for policy and service development.


Qualifications and Additional Requirements:

The post holder must hold an independent prescribing qualification and possess advanced knowledge of Mental Health patient pathways.

This Band 8b post is open to applicants with Approved Clinician Status or those developing towards it, as a Band 8b Annex 21 post, with a commitment to achieve Approved Clinician Status within an agreed timeframe. Appointments will follow the All Wales Annex 21 rules.

We serve Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties with over 13,000 staff providing primary, community, hospital, mental health, and learning disabilities services. Our facilities include:

* Four main hospitals: Bronglais, Glangwili, Prince Philip, and Withybush
* Five community hospitals and two integrated care centres
* Numerous community services including general practices, pharmacies, ophthalmic practices, and mental health services.

This role includes a development plan, with staged salary progression over four years, supporting qualification achievement. Successful completion will lead to a full Band 8b salary. Support and supervision are provided, but failure to meet the requirements may result in re-deployment.
