Key Responsibilities Day to Day PCN Coordination (4 days per week) To work with and support the PCNs Clinical Director, Strategic Manager, PCN members and their Teams to ensure effective delivery of the PCN DES and IIF requirements Responsible for assisting with day-to-day operational delivery Support equity of service provision across the PCN Trouble shooting day to day and responding to any queries.

Coordination of PCN Board Meetings, Clinical Advisory Team (CAT meetings and any other meeting as and when requested.

Produce, review and circulate documents required for meetings, and take effective minutes, collating action points.

Develop and maintain the PCNs social media platforms including regular uploading of information to public audiences.

Support the PCN to develop/ maintain a PCN patient voice forum, Organise meetings or events related to PCN activity.

Where required, support project delivery.

This could range from sole PCN projects to wider neighbourhood work involving the PCN. Help develop joint working relationships across PCNs town wide and its stakeholders.

Communicate relevant information, including briefings and reports to all member practices on a regular basis.

Maintain excellent relationships across all core member Practices.

Support PCN members with all other reasonable administration requests.

Support and collate evaluation of PCN services to drive further improvements.

Support the financial delivery of PCN activities.

People waiting for assessments for neurodevelopmental pathways (adults) (1 day per week) You will support individuals and their carers / families who contact the practice in relation to neurodiverse pathway queries/assessments.

The role focuses on providing practical, emotional, support to the patient, along with supporting administrative work at practice level to ensure patients are supported during waiting periods for assessments Key responsibilities Provide support to the practice to ensure documentation and coordination of referral pathways to ensure the patient is accepted for assessment.

Maintain and track a caseload of patients who may require support in the waiting period.

Have a positive, empathetic and responsive conversation with the person and their family and carer(s) about their needs.

Work closely with Practice Managers, GPs, social prescribers, and mental health teams Patient & Family Support Act as a named point of contact for practices Communicate with empathy, when dealing with patients and their carers Signpost to local and national support services (e.g. charities, peer groups) and develop a PCN directory of contacts.

Assessment Preparation Support practices by completion of forms at practice level for referrals Support patients and families to complete appropriate referral questionnaires and forms Liaise with Carers, or other professionals where appropriate Communication & Liaison Act as a bridge between primary care, specialist services, and or under the right to choose pathways Communicate with empathy for non NHS referrals Provide updates on referral progress where available Support patient understanding of the pathway Record Keeping & Administration Accurately document patient contacts and interventions in GP clinical systems Maintain up-to-date records of referrals and outcomes Support data collection for service evaluation and improvement Personal and Professional Development Keep abreast of national and local guidance on PCNs Take responsibility for own development, learning and performance.

Participate in an annual individual performance review, and take responsibility for maintaining a record of own personal and professional development Collaborative Working Developing and maintaining effective working relationships both within and outside the practice, PCN and locality Liaise with other stakeholders as needed for the collective benefit of patients Assist the PCN to achieve successful MDTs across South Neighbourhood Other Duties and Responsibilities Support the Network with coordination of Clinical projects in line with the PCNs priorities.

As and when required help people to manage their needs through answering queries, making / managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care As and when required provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; and support the coordination and delivery of MDTs The NHS is due to issue a new Contract and new care models, it is expected that where appropriate this role will help the PCN to deliver elements of the Integrated Neighbourhood Teams (INT) at this stage it is unknow what this is until the Contract is issued for 2026/

Personal Management Recognise and work within own competence Prioritise, organise and manage own workload in a manner that maintains and promotes quality Demonstrate understanding of the implications of national priorities for the PCN Keep abreast of the PCN Network Contract DES requirements and associated service specifications Bring to attention of the Strategic Manager and Clinical Director any clinical incidents, and participate as required in any investigation and learning exercise Bring to attention of the Strategic Manager and Clinical Director any formal complaints, and participate as required in any investigation and subsequent learning exercise Education, Training and Development Participate in relevant training programmes Maintain a statutory and mandatory training record
