Job Summary
We have an exciting opportunity for a Care Coordinator to support in our clinical service delivery. Many of our clients have complex needs and this role ensures that clients receive mental health assessment and important aspects of the clinical pathway as well as liaison with other internal and external services as needed.

Qualifications
Essential
Accredited Mental Health Nurse or Social Worker or Occupational
Therapist with significant experience of working in the mental health
sector with complex mental health presentations.
Registration with the appropriate UK governing body eg Social Work
England, HCPC, NMC, Scottish Social Services Council
Substantial post registration experience
Evidence of post registration development
Knowledge and experience of the Care Coordination approach
Previous experience of working in community mental health settings in a similar role
Advanced Assessment Skills
Demonstrate an extensive knowledge of evidence based assessments and interventions
Advanced knowledge and experience of safeguarding procedures
Understanding of the work of Freedom from Torture and complex trauma presentations
Experience of working under pressure, demonstrating the ability to work in a flexible way that is dependent on the circumstances that are
presented
Team player able to work within a small team
High level of psychological resilience to be able to work with challenging and distressing content
Able to challenge and be challenged
Desirable

Experience of working with refugees or asylum seekers and awareness of
the needs of this client group
