Job summary
We are seeking a compassionate and dedicated Healthcare Assistant to join our community team in Hampshire and work towards providing holistic, person centred, end of life care and support needs of people in their own homes.
Main duties of the job
Your responsibilities will include monitoring and assisting with the maintenance of patients' skin integrity and personal hygiene, observing and supporting nutritional needs, attending to comfort requirements to facilitate rest and sleep, providing mobility support, and maintaining a clean and orderly care environment.
As a Healthcare Assistant you will practice empathetic communication with the patient and those important to them in a supportive, professional manner and provide high quality personal care and support to enhance wellbeing.

Work Schedule:Monday to Sunday, including bank holidays
Shift hours: 8:00 AM to 2:00 PM and 4:00 PM to 10:00 PM
Salary: £24,465 per annum(prorated), plus enhancements
Based: Community Based - Hampshire
Whatwere looking for:
* At least six months of recent experience providingcare in a Health or Social Care Environment, including caring for patients atthe end of life.
* NVQ Level 2 in Health and Social Care or equivalent(preferred, not required).
* Be person-centred and compassionatewhen caring for the person and those important to them.
* Ability to develop and evaluate caremanagement plans that meet the complex needs of the person, and those importantto them.
* Emotionalresilience and the capacity to work independently.
* Avalid driving license and access to a car for travel to patient homes arerequired.
